Surrey residents only: order a self-testing kit
There are two types of home kits available to Surrey residents:
- Sexual health home screening kit: If you are over 16, live in Surrey (you will have to enter a Surrey County Council area postcode) and have no symptoms, you can order a home self-test kit for a full STI (sexually transmitted infections) check-up. This includes a test for Chlamydia, Gonorrhoea, HIV and Syphilis. Visit our homepage to order.
2. Chlamydia and Gonorrhoea screening service: If you are under 25, live in Surrey and only want a test for Chlamydia or Gonorrhoea, use the form below to order a free self-test kit.

It's free, confidential and easy to use; only a urine sample is needed.
To order a self-testing kit, simply complete the online form below or text the word REACH, followed by your full name, address and date of birth to 80010.
Your self-testing kit will arrive at the address you provide in discreet and plain packaging. If you prefer to request a kit by phone call the Chlamydia Screening Outreach Team to request a kit on 01483 846421.
